Permission To Transport Day Care Children
I give permission to Ashante Oliver and the staff at Magic Sitter Child Care to transport my child _____________________, DOB _____________, off the daycare premises either by vehicle or on foot to (check all that apply)

NY State Law requires that a child under the age of 7 ride in a car seat or booster seat. The car seat or booster seat must be appropriate for the weight and age of the child.

Each Child in the vehicle must have their own seat and seat belt. Each Child will be buckled properly in their seat.
The driver of the vehicle is at least 18 years of age, has a valid NY State drivers license, current registration and insurance.
